SETUP INSTRUCTIONS

Please follow the following simple instructions to setup your Internet access service.

Setting up your Internet Access Account is a very simple process.  Simply provide the information requested in the form below.   If you ordered this service by credit card, the order number is the last 9 digits shown under "Order number" in your "Order confirmation" page which you should have printed.  It also appears in the authorization e-mail that you should have received (or should receive when you check your e-mail).  If you ordered by Fax/Mail, the order number is stamped on your form.The items in blue are required.  The items in black are not because we should have received that information with your credit card, fax/mail, or phone order.

The desired login name you provide must be between 4 and 8 characters in length.  It should not consist of simple names such as "mary" or "steve" but of a combination of the first and last names such as "maryw" or "marwhi" (for Mary White) or any other combination you can think of.  Digits are allowed as well.  The desired login password must also be between 4 and 8 characters and it should be something that you can remember easily such as your birth date, family member's name, etc.
